Bladder distention from filling results in: (1) a-adrenergic contraction of the urethral smooth muscle and Increased tone on the veslcal neck via the Tl 1-L. Urethral coaptation results in part from filling of the rich subepithelial vascular plexus. The striated urogenital sphincter lies external to the urethral smooth muscle layers. These receptors arc stimulated by norcpinephrinc, which initiates a cascade of events that prefer� cntially results in urethral contraction and aids urine storage and continence. The urocpithelium is supported by a connective tissue layer, which is thrown into deep folds, also called pliclltiom. In ladies who arc hypoestrogcnic, this submucosal vasculaturc plexus is much less prominent. In part, local hormone substitute tar� will get this diminished vascularity and, in principle, enhances coapta� tion to enhance continence. Of the muscarinic receptors, five glycoproteins designated M 1-Ms have been recognized. M2 and M3 receptor subtypes arc prcdomi� nantly liable for dctrusor smooth muscle contraction.
